Heart Therapy Center

Patients receiving heart care at Valley may be referred to Valley’s Heart Therapy Center for treatment. The Center provides a unique outpatient setting where patients can receive medication management, infusion therapies, and coordinated care tailored to their needs.

With 14 infusion rooms and close proximity to The Valley Hospital, the Heart Therapy Center offers convenient access to care for patients transitioning from inpatient to outpatient treatment. The team's goal is to help manage symptoms, improve quality of life, and reduce the risk of hospital readmissions.

The Valley Approach to Heart Therapy 

The Heart Therapy Center works closely with each patient's cardiologist and specialists to ensure continuity of care. This coordinated approach means that all clinicians involved in a patient's treatment remain informed and aligned on the care plan.

The team assists patients in coordinating timely appointments to support prompt evaluation and treatment.

The Center provides care for patients with Left Ventricular Assist Devices (LVADs) implanted at Valley Hospital or other centers using a shared-care approach maintaining collaboration with their implanting center.

Medication Management

The team provides medication management for patients with:

• Advanced heart failure
• Heart failure
• Hyperlipidemia 
• Cardiac amyloidosis
• Chronic home infusions of heart failure medications
• LVAD implants

Infusion Therapy

Patients may receive medications and infusions specifically tailored to their treatment needs in one of the Center's 14 infusion rooms.

Telephone Triage

Patients can call the Heart Therapy Center to:

• Discuss symptoms
• Have medications adjusted
• Schedule an appointment
• Receive guidance from the clinical team

Remote Monitoring

Nurses closely monitor patients through implantable heart failure devices and can interrogate implantable defibrillators/pacemakers when clinically indicated.

Implantable Loop Recorder Placement 

For patients who are identified to need an implantable rhythm monitor to detect arrhythmias, they may be referred to this site to obtain an implantable loop recorder.   

Home Infusion Oversight

The team provides ongoing oversight for patients receiving continuous home infusions for heart failure symptoms and can adjust medications remotely as needed.
