At Valley, we recognize the need to provide care to our patients wherever possible. With the increasing demand for care to be provided in the home, Valley Medical Group offers Home-Based Primary Care to meet the needs of the community.
We provide primary care services to patients at their residences. Our services include the following:
- Routine primary care visits
- Prescription medication management
- Blood work and laboratory testing
- Coordination with imaging companies to provide orders for portable EKGs, X-rays, ultrasounds, and more
- Coordination with other Valley Health System resources to provide home care services like physical and occupational therapy, speech therapy, and wound care management
- Annual wellness visits
- Availability for urgent-care visits and telehealth visits

Who is Eligible for Home-Based Primary Care?
Valley’s Home-Based Primary Care program is designed for patients who face challenges traveling to their primary care provider. The best candidates include those with mobility issues, difficulty leaving their residence, or other access barriers that make in-office visits challenging. Services are currently available to eligible patients residing in select assisted living and senior communities, as well as in towns across Bergen County and the surrounding area.
Valley's Home-Based Primary Care services are currently offered to residents in the following communities:

- Brightview in Paramus
- The Bristal in Englewood
- The Bristal in Waldwick
- CareOne at the Cupola in Paramus
- Christian Health Care Center – Longview in Wyckoff
- Christian Health Care Center – The Vista in Wyckoff
- Harmony Village in Paramus
- Juniper Village in Paramus
- Sunrise in Franklin Lakes
- Sunrise in Wayne
- Sunrise in Woodcliff Lake
- Thrive in Montvale
- Van Dyk in Hawthorne
- Windemere in North Haledon
