Geriatric care is a medical specialty that focuses on the healthcare needs of older adults. It involves prevention, treatment, and diagnosis of various health conditions that this population may face. Care can include both medical and personal needs, helping to promote increased independence and greater comfort in daily life.
Geriatric medicine focuses on those who are age 65 and older, but many patients that require this type of expertise are in their 80s and beyond.

At Valley, geriatric care goes beyond managing medical needs. Our dedicated social worker partners with patients, families, and physicians to help navigate the complexities of aging. From connecting you with community resources and financial assistance to coordinating care, supporting caregivers, and advocating on your behalf, we provide personalized support to help you stay healthy and independent at home.
In addition, Valley CARES provides information on support services and programs, both within Valley and at the local/state/federal level, that can assist patients in getting the care they need, where and when they need it.
Our geriatric care providers are fellowship-trained and provide office-based, outpatient-focused geriatric assessments. They work in conjunction with the hospital team to provide the transition of care from the hospital setting to other outpatient and home settings.
