For those 18 and older, a Zocdoc account is required to schedule your vaccine appointment. For faster scheduling, create your account in advance and have your password handy when you make your appointment. Click here to create an account on Zocdoc.
Appointments for those under age 18 must be made through a parent or guardian Zocdoc account.
Once at the "Review and book" step, select "Someone else" when asked who will be attending the appointment. Then, enter the information of the individual who will be receiving the vaccine and select "Yes" if you are the parent or legal guardian of this patient. See the example below:
