What is Valley’s Hospital at Home Program?
Valley's Hospital at Home program brings the same high-quality, hospital-level care you would receive in our Paramus hospital, right in the comfort of your own home.
The program provides appropriate patients with daily clinical assessments, 24/7 monitoring, medical supplies, and home visits from nurses and specialists so they can more quickly and successfully heal in the comfort of their own home. With daily virtual clinician visits, in-home nursing visits, remote monitoring, and clinical support, patients receive the same high standard of care that they would receive in the hospital.
How Does the Hospital at Home Program Work?
- Based on your condition, the physician overseeing your care at The Valley Hospital – your hospitalist – will make a referral to Valley’s Hospital at Home program.
- A member of the Hospital at Home team will assess your clinical stability, your available support and safety at home, and check your insurance coverage to ensure this care is right for you.
- If an assessment identifies that you are eligible for Hospital at Home care, a member of our team will visit your home to deliver and install medical equipment and arrange for your first virtual check-in appointment.
- Throughout your entire Hospital at Home stay, you will have two daily nursing or paramedic visits, and one virtual clinician visits assessing your clinical status. Well-defined escalation protocols will be followed should your condition change.
Am I a Candidate for Hospital at Home Care?
Upon being admitted to the hospital, the doctor in charge of your care will conduct a clinical assessment using established medical criteria to decide if you can receive the care you need at home using Valley’s Hospital at Home program.
You are considered a candidate for the program if:
- You meet the medical criteria to safely receive hospital-level care at home.
- You have a safe home environment that supports your recovery.
- You or a caregiver can participate in your care as needed.
