Starting February 27, 2012, the shuttle will no longer pick up or drop off riders at the Luckow Pavilion, located at 1 Valley Health Plaza. Please do not park at the Luckow Pavilion. The shuttle will pick you up and drop you off at 611 Valley Health Plaza, which is the parking lot next door to Luckow. Please park in this parking lot only. Thank you for your cooperation.

As a reminder, please run your high, lo glucose controls and complete the annual competency test by the end of February 2012. Please drop the signed competency form and statement of confidentiality to Beverly Karas-Irwin's office.

Dear Clinical Faculty:
Thank you for choosing Valley Hospital for your clinical rotation. Our goal is to provide a safe environment for you and your students.
Unit-specific information, such as manager's name, email address, and telephone number, can be found by clicking on Unit Manager Contact Information.
All faculty and students need to complete required paperwork annually, on a calendar-year basis. Please see the new mandatories below as of January 2012 under the "Required Paperwork" section.
It is imperative that we receive all required paperwork, including the School Affiliation Roster, by the end of your first clinical day. Please return the paperwork to Cathy Hamilton, 4 West. Incomplete paperwork cannot be processed, and will be returned to you for completion.

Required Paperwork
a) School Affiliation Roster - Please refer to the School Affiliation Roster Example on how to complete. Student grades to be noted on roster. A Student Affiliation Roster must be submitted for all clinical rotation groups. If a student is returning within the calendar year, please mark “Returning student.”
b) Students & Supplemental Staff Self Learning Orientation Module Post Test - Please note:
- Read each section.
- Complete the test (last section of the book) using the answer sheet.
- Complete the Infection Control Post Test.
- Sign the "Student/Faculty Statement of Confidentiality."
- Sign the "Statement of Understanding and Compliance with the Valley Hospital's Code of Ethics."
c) Products for Sharps Safety - To ensure review of safety products.
d) Mandatory Insulin Education -
For Instructor Information/Completion or for Review with Students
a) Personal Appearance Guidelines - We ask that you review with your students.
b) Nursing Student Clinical Assignment Sheet - Must be filled out and posted daily on the unit.
c) Post Test Answer Sheets - For grading and reviewing results with your students.
