Valley Home Care offers a Diabetes Self-Management Education program for people who are interested in learning how to manage their new diabetes diagnosis or better manage their current diabetes status. The program is a four-week course with two-hour meetings once a week. Each participant is invited to bring a family member or guest for support. Prior to participating in the program, registrants must complete a one-hour individual assessment with a diabetes educator.
Valley Home Care's Diabetes Self-Management Education Program has been recognized by the American Diabetes Association for Quality Diabetes Self-Management Education* and Support. Valley's program has received this recognition consecutively since 2001.
Class topics include: diabetes knowledge and self-management skills assessment, monitoring and use of blood glucose results, nutritional management, carbohydrate counting, medication review, changing behavior, coping & psychosocial adjustment, foot care, hypoglycemia/hyperglycemia, exercise and community resources.

Many insurance companies, including Medicare, will cover this program as a benefit to their subscribers. The out-of-pocket costs for the program will depend on the policy coverage and any limitations. The program is covered for all Valley Health System employees enrolled in the System's health plan.

Diabetes Support Groups (Adult and Peds Group)
Adult Diabetes Support Group
This group meets on the fourth Thursday of the month (excluding August and December) at 7 p.m. at the Kraft Center, 15 Essex Road, Paramus.
Pediatric Diabetes Support Group
This group meets on the second Tuesday of the month (excluding July and August) at 7:00 p.m. at the Kraft Center, 15 Essex Road, Paramus. The support group is open to children with diabetes under age 18 and their families. Siblings are welcome. Every month there is a planned speaker or activity.
