MLTSS (Managed Long Term Services and Supports)

For individuals who need assistance activities of daily living in the home on a long term basis, Medicaid eligible persons can receive services in the home as a way to prevent institutionalized care, such as a nursing home. MLTSS enables a beneficiary to live in the community with long-term supports for as long as possible. Further, MLTSS provides comprehensive services and supports, whether you live at home, in an assisted living facility, in community residential services or in a nursing home. In order to be eligible for this service, individuals must meet certain criteria (financial, medical need and age and/or disability). Visiting Homemaker of Bergen County is a private, non-profit provider of home care services that provides a spectrum of services that meet an individual's needs from live-in care to "occasion care," which is accompanying an elderly or disabled person to a special event. They also have several free programs, one being Cheer, which offers seniors with assistance with shopping, errands, laundry and socialization.
