Getting Started with Valley Hospice
For questions about hospice or our program, please call 201-291-6131 and a Valley Hospice representative will be happy to assist you. A physician’s order is necessary to start hospice care; however a member of our team can do an evaluation to determine appropriateness for hospice before attaining the physician’s order.
Community Liaison: Joanna Hannon, BSN, HN-BC
Joanna is a graduate of Felician College and has been with Valley Home Care & Hospice for over 25 years. She is an expert in hospice eligibility and can assist in determining if hospice is appropriate for the patient and family.
Hospital Liaison: Ana Cernadas, RN
Ana Cernadas graduated from UMDNJ/ECC school of nursing. She has over 20 years of experience in hospice care in different capacities. Ana is an expert in hospice eligibility and she will be available to meet with patients and families to explain hospice benefit and to help determine if hospice is appropriate for them.
Once a referral is received, a Hospice representative will contact the patient, family and/or caregiver to initiate the first visit from the Hospice Nurse who will evaluate the patients’ needs and develop a plan of care if the patient meets the Hospice requirements.
