A Living Will is a statement of your wishes at a point in time when treatment options are no longer helpful in successfully treating mental or physical diseases.
The Living Will allows you to state your wishes regarding treatments such as artificial feeding tubes, CPR, respiratory support, or hemodialysis. The Important Decisions Brochure (which can be viewed and printed below) can help you understand your choices.
The form includes two parts: a Health Care Proxy and the Advance Directive. After the form is filled out, you will need to have it witnessed by two people. Please note that the person you designate as your Health Care Proxy CANNOT act as a witness.
