As part of our efforts to enhance efficiency and environmental sustainability, The Valley Hospital's Health Information Management department will be going paperless effective April 1, 2024. All requests for medical records should now be submitted electronically using the links below, as we are no longer accepting paper requests.

Valley offers an online records request tool that helps us verify your identity by asking for a photo of your driver's license, which you can take via webcam or smartphone.

Diagnostic Images 

Electronic access to your images is available through the Ambra image sharing platform. Through this application, you can view, download, and share your images.

AMBRA PORTAL LOG IN

In addition, diagnostic images can be obtained on a CD or shared with you through Ambra via email.

Valley has direct image transfer connections with the following organizations. Image requests do not require a CD to transfer images prior to your appointment:

  • NewYork-Presbyterian
  • Cleveland Clinic
  • The Mount Sinai Health System
  • Hackensack Meridian Health, Hackensack University Medical Center
  • Memorial Sloan Kettering Cancer Center
  • Weill Cornell Medicine

Images can also be directly shared with your physician or physician's office as needed.

Diagnostic images available include X-ray, CT, PET, MRI, nuclear medicine, breast, and echo images.

Personal Information Changes

Personal information changes must be submitted in writing, with a copy of your valid driver's license, to the Health Information Management Department.

Birth Certificates

To obtain copies of birth certificates, please call the Ridgewood Board of Health at 201-670-5553. The Health Information Management Department does not keep any information on birth certificates.
